Discharge from Enrollment:
- Search for the Participant
- Or select the Participant from the program listing
- Navigate to their Demographics screen so that you can see the yellow bar along the top of the TREAT session (click Demographics on the left sidebar if you do not see the yellow bar right away)
- You can see that this Participant is Enrolled in one or more programs
- Each program shows a different primary worker – selected on the Enrollment Encounter
- You can l proceed to close the program where it shows the primary worker’s name
- Navigate to their Encounters screen so that you can see the yellow bar along the top of the TREAT session (click Encounters on the left sidebar)
- You can see that this Participant has many Encounters (Encounter Visit Types are either “Visit” or “Enrollment”)
- You have to locate the Enrollment record for the specific program name (Enrollment (encounters) are usually located on the last page because they appear in descending date order)
- Scroll down to the bottom of the screen
- Select the drop-down beside Page and select the last option
- Or click “>>” to jump to the last screen
- Click Edit on the same row as the specific program name where the Visit Type is Enrollment
Note: An Appointment or Progress Note can be Scheduled or Attended even when the Participant is not Enrolled. You will see an alert/warning message notifying you this is the case.
Program Enrollment Information Status
- Open
- Closed
- Cancelled
End Date
- use calendar icon
Closed Reason:
- Completed Program/End of Session
- External Transfer – Other than Hospitalized
- Internal Program Transfer
- Drop Out
- Client Withdrew and Notified Staff
- Terminated by Staff – Unvoluntary Discharge
- Incarcerated
- Deceased
- Hospitalized
- Mutual Agreed Upon Termination
- Extended Period of No Contact
- Family/Client Moved
- Other
- Unknown
- Enrolled in Error
- Start Job
- Start School
14. Change the Status to Closed
15. Use the calendar to enter the Closed Date – use a future date if needed
16. Select the Closed Reason
17. Click Save
18. If there are other Programs open for this Participant you will see a warning appear.
19. Click Ignore Warning(s) and Save
20. Now you can see that the Status is Discharged
21. Now you see on the yellow bar for this Participant that the Discharged program is no longer there
22. Navigate to your Homepage (click OCASE in the top left corner)
23. With the Participant list selected, you can see there is now one less Participants and this list does not show your/the client as a member in the Program
24. One additional bit of information – if the reason for a discharge was Deceased then you would also want to navigate to the Demographic screen and click Edit.
25. Known Date of Death = Known, Estimate, Unknown
26. Date Deceased – use the calendar to enter the information in this field
27. Click Save
