When you create a tracking tool entry in OCHART, it is best practice to give that entry a unique title.
For example, instead of titling the entry for a presentation you gave as "HIV 101 Presentation", you could title it "HIV 101, Presentation at local community centre".
If you are tracking an activity that is repeated, you could consider adding a session number to the title to make the title unique: "Drop-In Group Session 2".
Please ensure that titles and the tracking tool entries themselves do not contain any confidential or identifying information.
